I understand that the services I will be receiving are not offered as a subsititute for professional mental health care or medical care and are not intened to diagnose, treat or cur any mental health or medical conditions. I also understand that my coach i s not acting as a mental health counselor or a medical professional. If you require medical or psychological services, it is your responsibility to seek out the attention of a licentsed doctor or mental health professional.

I understand that my coach will protect my information as confidential unless I state otherwise in writing. If I threaten to harm myself or someone else, I understand that necessary actions will be taken and my confidentiality agreement limited in this capacity.

I acknowledge that I am voluntarily participating in the sessions provided by THE PULSE. I understand that phusical excercise by itts very nature carries with it certain inherent risks, inclusing but not limited to physical injury, strain, discomfort and even the possibility of serious injury. I hereby assume all risks and responsibility for any such injueires or other medical incidents.
